This is a valid Affidavit of Service for the courts and can be used in conjuction with a valid Proof of Service form for your case.  An Affidavit of Service is powerful tool to use to further verify that service was completed. The Affidavit provides a much more detailed explanation of the way service was completed. This is a signed and noterized document confirming services have been complete.
